Care Planning & Directives

Patients have the right and responsibility to control decisions regarding the use or refusal of medical care, including life-prolonging treatment, nutrition and hydration.

One way to control those decisions is by completing a healthcare directive. Healthcare directives, including those from residents of other states, are honored at Unity Medical Center and can be written while a patient is admitted.

Advance directive forms are available online or from the social worker.
